Background:
|
The JIP in Yorkshire and Humber was formed in early 2007, and over a period of six months developed the “People Matter” strategy. This has a number of key objectives aimed at allowing people to live their own lives as they choose using services which are high quality, safe and promote independence, well-being and dignity.
The central planks of the ‘People Matter’ strategy are Personalisation, Commissioning, Improving Performance and Efficiency.
This project is within the scope of the regional programme for Personalisation for the Yorkshire & Humberside which was originally developed by the In Control’s Regional Forum. The overall regional programme for personalisation provides a framework for responding to all the four quadrants of personalisation and offers a framework of distinct work-streams by which to drive forward commissioning and personalisation across the region.

Delivery:

1. Share and learn good practice across the region around the universal offer of information
2. Develop information frameworks across the region that can be shared and used locally to give a more personalised information service that incorporates best practice documents and signposting
3. Articulate and re-iterate the principle that transformation and personalisation are dependent upon good information and advice as set out in the IDEA report, Transforming Adult Social Care, Access to Information, Advice and Advocacy” February 2009
To deliver the aims of the project there are 3 elements that are embedded within it:
· Information provision – the development of best practice templates for use across the region
· Information template for recording individuals information needs
· Information directories which are support solutions for staff to deliver effective signposting solutions – enabling the individual to own their information needs

Outcomes:
|
· Shared learning/resources (Information provision templates, signposting and information directories providing an opportunity to look at good practice and avoid duplication of work and cost.
· Develop an information pathway that can be adopted across a range of sites - a pathway which offers a consistent approach that works regionally and locally which is based on the idea of offering " Information Prescriptions" to various client groups via
· Generic information prescription points, such as call centres, libraries, council information points, health sites and voluntary and community organisations
· This would deliver information prescription products, such as a standard prescription for self funders which would satisfy their information needs. For example signpost clients to a host of regional and local sites, materials/fact sheets about what info they need to make informed choices etc
· Information is developed to suit the needs of the person, rather than a group of people. Groups at risk of exclusion will be considered to ensure that content and strategies for disseminating information are appropriate for a wide range of needs. A pick and mix approach is available because of the range of partners involved.
· New approaches to information sharing, both receiving and disseminating, will be identified and utilised to test their feasibility in improving the ‘offer’ of access to universal information.
Success of this project will look like
· Information is based on the same ideas across the region
· Obtainable from a range of locations and not just ‘traditional’ information pathways
· Enable people to make informed choices
· Will be personalised to the individuals needs
· Support Councils across the region in delivering the four part ambition for personalisation
A web based regional network will be put in place to provide mutual learning and support and will be hosted by Kirklees Council, the lead authority. This will link into the Association of Social Care Communicators.
