Background:
|
The white paper Valuing People A new strategy for people with Learning disabilities in the 21st Century 2001, set out a challenging task for councils to modernise their day services by 2006. For decades, people with learning disabilities have been heavily reliant on large and often institutional day centres. These have provided much respite to families but they have made a limited contribution to promoting social inclusion or independence for people with a learning disability.
The Publication of Valuing People Now has re-emphasised the need for all Local Authorities to invest in making community based facilities and settings that are accessible for all.

Delivery:
|
Ø taking stock of and understanding the current situation, and identifying the ‘cohort’ of people affected by the proposed work – and then designing the work accordingly;
Ø Supporting each area to develop a change plan that mirrors the values set out in VP Now and Government policy.
Ø To assist the embedding of general awareness raising about personalisation in their plan and what it means for people with LD, their families, and staff currently supporting people in Day and residential services;
Ø Ensure that each local plan is linked to the setting new Adult Workforce Strategy and any local developments;
Ø using the links between this project and other groups and ‘drivers’ locally in each area’s Social Care Reform programme – and building this explicitly into the project plans;
Ø linking in with existing wider community initiatives to ‘open up‘ new employment, educational, and leisure ‘pathways’ and opportunities, etc – and building also into the project plans

Outcomes:
|
This project will provide the following outcomes. It will:
Ø Support each area to identify a range of resources that can be released from current services to support the development of personalization
Ø Identify, engage and make links to other citizenship and inclusion projects, the local strategic partnership and the Local Area Agreements
Ø To work out efficiency savings and re-investment of current resources linked to personalization.
Ø Develop detailed transitional plans
Ø Create a detailed project plan for change of current services in partnership with people and families
Ø To support each area to develop Workforce plans linked to the new support they are developing
Ø To support each area to ensure that people with complex needs are included in the move from traditional services to a personal support package
Ø To support localities to connect with others who share the same vision and are under going the same processes
Ø To support each area to link the learning from National programmes – Individual Budgets, POPS, Getting a Life project, In Control and Putting People First.
Ø The project will also make strong links, share learning and work in partnership with regional activities, including the personalization work plan for the region.
Ø The project will develop a transition plan for service development that can then be duplicated across other client groups facing similar issues, i.e Mental Health and Older People
