Background:
 The JIP in Yorkshire and Humber was formed in early 2007, and over a period of six months developed the “People Matter” strategy. This has a number of key objectives aimed at allowing people to live their own lives as they choose using services which are high quality, safe and promote independence, well-being and dignity.
 
The central planks of the ‘People Matter’ strategy are Personalisation, Commissioning, Improving Performance and Efficiency.
 
This project is within the scope of the regional programme for Personalisation for the Yorkshire & Humberside which was originally developed by the In Control’s Regional Forum. The overall regional programme for personalisation provides a framework for responding to all the four quadrants of personalisation and offers a framework of distinct work-streams by which to drive forward commissioning and personalisation across the region. 
 
Delivery: 
The overall aim is :
 
To co-produce a framework and key elements of that framework that will form the basis of delivering individualised support across health and social care – with a particular focus on health. 
 
 
  • To develop and test a Resource Allocation System for Continuing Healthcare that could be used across the Yorkshire and Humber Region
 
  • To develop a range of models for contracting and delivering personal health budgets.
 
  • To develop the operating principles and values with an agreed financial and contracting framework that will underpin the overall framework
 
  • To gather and share good stories from families and individuals.
 
  • To explore the principles that will underpin a clinical governance framework
 
  • To develop criteria and principles for signing off a support plan for meeting health needs
 
  • To develop a framework for reviewing personal health budgets in conjunction with social care
 
  • To develop approaches that systematically capture health and well being outcomes
 
  • To develop links to Personal Health Budgets pilots to ensure complementary activity/outcomes.
 
Outcomes:
 
Key outcomes include :
 
  • A Resource Allocation Framework for continuing healthcare
  • Guidance on different approaches to contracting support
  • An analysis of barriers and possible solutions
  • Good stories from families
  • An operating framework for health – including criteria for a good support plan, a clinical governance framework and a process for signing off and reviewing plans.
  • Information to PCT’s about how to organise support and an analysis of the barriers and legal issues.
