Background:
|
The JIP in Yorkshire and Humber was formed in early 2007, and over a period of six months developed the “People Matter” strategy. This has a number of key objectives aimed at allowing people to live their own lives as they choose using services which are high quality, safe and promote independence, well-being and dignity.
The central planks of the ‘People Matter’ strategy are Personalisation, Commissioning, Improving Performance and Efficiency.
This project is within the scope of the regional programme for Personalisation for the Yorkshire & Humberside which was originally developed by the In Control’s Regional Forum. The overall regional programme for personalisation provides a framework for responding to all the four quadrants of personalisation and offers a framework of distinct work-streams by which to drive forward commissioning and personalisation across the region.

Delivery:

The project will essentially have 4 key work-streams:
Ø To look at the core competencies required of Care Management/ Social Workers to support the 7 Step model of Self-directed support.
Ø To support Yorkshire and Humberside Regional Team /Services managers in supporting their staff in the transformation towards Self-directed support through a series of Action Learning Sets.
Ø To offer workshops across the Region in ‘Person Centred Approaches / Support Planning’ to begin to give staff confidence in this key area of supporting individuals in devising support plans.
Ø To support the work within the Workforce Development work steam, in understanding the worker role requirements for the future.
Ø To look at how client I.T. system can support the Self-directed support approach to achieve the C.Q.C 7 outcomes and meet performance criteria. This work will be looked at within the City of York, and share the learning across the region.

Outcomes:
|
To support the cultural shift towards a Self-directed support approach within Care Management and Social work. To establish the core competencies required to make this shift and to begin to give practitioners some resources / skills that will give confidence to work within this new approach, supporting the Workforce Development objectives.
An overarching aim will be to support the C.Q.C 7 outcomes.
